We think this amp dates to 1970-75. It is seen in the upper "shelf" of a 122RV or 147RV (and a few other -R / -RV Leslie models) It runs a pair of 7189 tubes, and features power plug, output connector, and RCA leads for the reverb tank (not included); this one also has what appears to be a level potentiometer.
As seen, the power transformer was replaced with one that was probably too tall for the upper Leslie shelf to accommodate to the transformer was oriented on its side. The functionality isn't affected: the amp powers up and passes signal.
